We are looking for an energetic and diligent person to join our great team in the following role based in Yarrawonga. This role would be suitable for professionals with experience in:

  • Aged and Community Care
  • Case Management/Care Coordination
  • Allied Health
  • Nursing

 Position Title  – Care Coordinator (Home Care Packages)

                            Part Time  Ongoing (20 hours a week)

Yarrawonga Location

Salary range and benefits: SACS 5 ($42.94 per hour) + Non for-profit salary packaging options

What's involved in the role?

We are seeking a Part Time (20 hours p/w) Care Coordinator (Home Care Packages) to support our growing client base in the Shepparton and surrounding area. This position will be responsible for supporting existing client and onboarding new clients in accordance with our Home Care Package Procedures. This role has the potential to expand into a full-time position.

About you:

While a tertiary qualification in Health, Community Services, Allied Health  or Case Management and an understanding of the Home Care Package program, is desirable, what is also important are the attributes that you possess that speak to our values.
