We are looking for an energetic and diligent person to join our great team in the following role based in Yarrawonga. This role would be suitable for any Allied Health, Nursing or Community/Aged Care Assessment professionals.

Salary range and benefits: SACS 5 ($40.64 per hour) + Non for profit salary packaging options

What's involved in the role?

We are seeking a Full Time Care Coordinator (Home Care Packages) to be the first point of contact for Home Care Package referrals and enquiries, coordination for all eligible home care package clients, and provide support to the wider service coordination team.

About you:

For this role, you will require a tertiary qualification and experience in case management. The role could appeal to nurses, allied health professionals, Aged Care/Disability professionals or Social/Community Services professionals that feel comfortable in coordinating our clients’ care. You will need well developed computer skills and great interpersonal skills. What is also important are the attributes that you possess that speak to our values. We want our clients to have a great experience with our people, and our employees need to have a great attitude, be friendly, willing to learn new things, and most importantly listen and understand what the client wants and needs.
