CARE COORDINATOR – COMMONWEALTH HOME SUPPORT PROGRAM
Yarrawonga, Full Time Temporary position, up to 14 months Maternity Leave replacement

WHAT'S INVOLVED IN THE ROLE?
We are seeking a Full time Care Coordinator to be the first point of contact for all the incoming referrals of the Commonwealth Home Support Program (CHSP) at MHA Care.
To be successful you will need to be able to coordinate the care for all eligible CHSP clients, including but not limited to, intake, referrals, enquiries and providing support to clients (and their families) as well as the wider service coordination team.
While a tertiary qualification in Health, Disability or another related profession is in our essential criteria, what is also important are the attributes that you possess that speak to our values. We want our clients to have a great experience with our people, and our people need to have a great attitude, be friendly, willing to learn new things, and most importantly listen and understand what the client wants.
